Once we’ve recieved your referral, you’ll be put in touch with our Support Coordination Team Leader, who will guide you through our process and organise a first appointment. During this appointment, we’ll discuss your NDIS plan with you and determine what supports have been provided for as well as what connections are required. From here you’ll be connected with an appropriate support coordinator within Exaudy Care, and put in contact with the support you need.
